Output and defects
Output records good products and defects separately. A defect has a type, a decision and a culprit, so losses are visible by reason, not as one line at the end of the month.

What is included?
- Good products and defects, output serial numbers
- Operations, performers, labor hours and machine hours
- Correctable and irreparable defects
- Decision: rework, write-off, discount, return to supplier
- Culprit: employee, supplier, equipment or technology
- Work done by performer for piece-rate pay
